In order for the Belmar Youth Club (BYC) to succeed, we need each child’s parent or guardian to chaperone AT LEAST 2 events. On the back of this form, please number your top 4 dates to chaperone (1 being your first pick). We will do our best to accommodate your schedule. Chaperones should arrive 15 minutes before the event starts and be prepared to stay until all of the children have been picked up and the gym/pavilion is put back in good order.

FEES:
Registration – $10 on or before September 14th ($15 thereafter)

Chaperone Opt-Out (optional) – $50
